The following documentation is need to register your child for the 2019-2020 school year.
Proof of Identity - the parent/guardian is to provide a photo ID
Birth Certificate - Please bring in your child's original birth certificate. We will photocopy it for our files.
Proof of Residency - Proof of two of the following documents is required: utility bill, signed lease/rental agreement, settlement document on a new home, property tax bill, property deed, notarized letter from builder or realtor. Please note that automobile registrations, driver's license, or town resident form are NOT accepted.
If you are living with a resident of Kingston, NH, that resident must provide the acceptable residency documentation listed above and complete a Notarized Residency Affidavit by the Superintendent's office. The Residency Affidavit will list all family members living at that address.
Physical & Immunization Records - Each child is required to have a complete medical examination within one year prior to entry into school. NH Law requires all children to have had immunizations for DPT, Polio, Mumps, Measles, German Measles, Chicken Pox, and Hepatitis B prior to entry to school. If your child has not had a physical exam within this time, you must make an appointment as soon as possible. Your child will be allowed to enter school upon confirmation of the appointment.
Custody/Restraining Orders - It has become necessary for us to address the legal issue of custodial rights, restraining orders, etc. If there is a restraining order or limited access rights against either parent, it is the responsibility of the custodial parent to supply us with a copy of the legal paperwork stating this. This document will be placed on file until further notification. Please be advised that unless we have a copy of the legal documentation in our possession, we will release your child to either their natural parent or to anyone else that you give written permission to.
Release of Record Form - This form allows us to get your child's previous school records. You will need to print it and send it back to us.

Parent Residency Affidavit Form - This form is only needed if you cannot provide the required residency documents. Print this form and have the person you are living with complete and provide residency documentation to our Sanborn Regional School District office.

Placement Information Request Form - Please provide us with any information you believe might help us place your child in the most appropriate classroom setting. Please note that we consider many factors when deciding on the best placement for students and cannot honor specific teacher requests.
