REGISTRATION INFORMATION FOR THE 2020-2021 SCHOOL YEAR
Welcome to our Bakie School Community!
The first step is to complete one of the notification forms depending on your child's grade they will be entering.
Notification of New Student - Grades *Preschool, 1, 2, 3 - *Preschool students must be accepted into the program. We do not offer preschool to all residents. Acceptance is done via a lottery held in Jan/Feb. timeframe.
Notification of New Kindergarten Student
The next step is to provide the following documents to register your child.
Proof of Identity - the parent/guardian is to provide a photo ID
Birth Certificate - Please provide us with a copy of your child's original birth certificate. You will get an email so that you can upload the document securely.
Proof of Residency - Proof of two of the following documents is required: utility bill, signed lease/rental agreement, settlement document on a new home, property tax bill, property deed, notarized letter from builder or realtor. Please note that automobile registrations, driver's license, or town resident forms are NOT accepted. You will receive an email so that you can upload the documents securely or you may email them to email@example.com.
If you are living with a resident of Kingston or Newton, NH, that resident must provide the acceptable residency documentation listed above and complete a Residency Affidavit Form which must be notarized by the Superintendent's office. The Residency Affidavit will list all family members living at that address. Please schedule an appointment by sending an email to Phyllis Kennedy at firstname.lastname@example.org to have your document notarized.
Physical & Immunization Records - Each child is required to have a complete medical examination within one year prior to entry into school. NH Law requires all children to have had immunizations for DPT, Polio, Mumps, Measles, German Measles, Chicken Pox, and Hepatitis B prior to entry to school. If your child has not had a physical exam within this time, you must make an appointment as soon as possible. Your child will be allowed to enter school upon confirmation of the appointment. You will receive an email so that you can securely upload the documents into our portal.
Custody/Restraining Orders - It has become necessary for us to address the legal issue of custodial rights, restraining orders, etc. If there is a restraining order or limited access rights against either parent, it is the responsibility of the custodial parent to supply us with a copy of the legal paperwork stating this. This document will be placed on file until further notification. Please be advised that unless we have a copy of the legal documentation in our possession, we will release your child to either their natural parent or to anyone else that you give written permission to.
Release of Records Form - This form is needed to obtain your child's records from their previous school. Please print, complete, and email to email@example.com.
The following registrations forms are required to be completed online:
Online Registration - Click on this link to register your child. You will need at least one emergency contact and phone number to complete this. If entering Preschool, use grade 13. If entering Kindergarten, use grade 14. THIS IS FOR NEW STUDENTS ONLY. If your child attended Memorial School last year, you do NOT need to complete this registration.
Verification of Residence Form - click on this form and submit information online. Proofs must be provided in addition to submitting this form. You will receive an email where you can upload the proofs online.
Home Language Survey Form - click to complete for EACH student and submit online.
Emergency/Medication Permission Form - click to complete for EACH student and submit online
Student Transportation Form - click to complete for EACH student and submit online. New preschool enrollments do not need to complete this form.
Internet Access Permission Form - click to complete for EACH student and submit online
Military Connected Parent/Guardian Form - this form required by U.S. Dept. of Education. Complete for EACH student. THIS IS A NH STATE REQUIRED FORM. Please complete even if it does not apply.
Universal Field Trip Permission Form - click to complete for EACH student and submit online
Placement Information Request Form - Please provide us with any information you believe might help us place your child in the most appropriate classroom setting. Please note that we consider many factors when deciding on the best placement for students and cannot honor specific teacher requests.
