Immunizations          
 

In addition to the basic series of childhood immunizations, the following are requirements for middle school students.

**In accordance with NH Immunization Law, every parent or guardian of a child to be enrolled in a New Hampshire school shall, prior to his admittance, provide documentary proof of acceptable immunization.       
  Entering 6th Graders:

Must provide proof of varicella (chicken pox) immunity, documented by one of the following: 

  • Date of the disease provided by a parent/guardian or healthcare provider,
  • Date of receipt of the varicella vaccine, or
  • A lab test indicating varicella immunity. 
  Hepatitis B Vaccine:  If your child was born on or after 1/1/93, three doses of Hepatitis B Vaccine are required, administered at the appropriate intervals.   I  
 
Entering 7th Graders

Must provide documented proof of receiving a second measles immunization (MMR) or proof of measles immunity prior to the beginning of the school year. 

Note: The first measles immunization must have been administered after 12 months of age in order to be valid.

  Entering 8th Graders

Must provide proof of a tetanus booster immunization within the past 10 years.
