Medication/Allergy Forms

 
All students requiring regularly scheduled medications or any as-needed medications (such as inhalers, short-term antibiotics, etc.) need to have your permission for those meds to be administered in school. This includes students with allergies who require benadryl, an Epi-pen, or other medications or treatments.

These forms are available at the school or can be downloaded and printed from your computer by following the links below.

If you have questions about the medication administration policies, these can also be accessed through the links below. Remember that students are not permitted to carry medications to and from school -- this is for their safety and for the safety of the other students.  An adult must be the one to drop off and pick medications, which may be left at the front office if the Health Office is not open.
