MEDICATION ADMINISTRATION |

Students requiring medications to be administered during the school day must visit the school nurse. To ensure students' personal safety, the following policy has been established by the Sanborn Regional School Board (Policy JLCD) regarding medication administration to students:

| 1. All medications must be delivered to the school directly by the parent or guardian. Students should NOT be carrying their own medicine to school. | State law requires the school to keep a record of all medications administered in school | |
| 2. Medications must be delivered in the original pharmacy container. The label should show the child's name, date of prescription, name of drug and strength of medication, dose prescribed, and the physician's name. | Parents must sign a permission slip for the administration of any medication, either prescription, or over-the-counter medication. |
| 3. The medicine must be accompanied by a note from the prescribing physician stating that the medicine must be given during school hours | Prescribed medications should not be taken during school hours if it is possible to achieve the medical regimen at home. |
