IMMUNIZATION REQUIREMENTS 2006 - 2007 |

The table below lists the immunization requirements for entry into Memorial Elementary School. These immunizations are required by the New Hampshire Department of Public Health and the New Hampshire Department of Education. Each immunization has minimum acceptable intervals. This page can be printed and presented to your child's primary care provider at the time of his or her physical, to ensure that your child has been adequately immunized. < 7 Years of Age |
Four doses, 4th dose on or after 4th birthday or Five doses regardless of age of administration as long as minimum intervals are met. |

7 Years or Older |
Three or four doses, with last dose on or after the 4th birthday or Five doses as above. | |

POLIO: |
K - 12 |
Three doses of an all eIPV or all OPV schedule, with last dose after the 4th birthday. |
| Four doses of any combination of eIPV and/or OPV regardless
of age of administration. When a combination of polio vaccines have been administered, 4 doses are necessary even if the 3rd dose was administered after the 4th birthday. |

MEASLES |
K through 4th grade |
Two doses of measles-containing vaccine.
Dose 1 on or after 12 months, and Dose 2, a minimum of 28 days from 1st dose. |

7th - 12th grade |
Two doses of measles-containing vaccine. | |

All grades not mentioned above |
One dose. | |

RUBELLA AND MUMPS: |
K - 12 |
One dose of rubella and mumps-containing vaccine administered on or after age 12 months. |

HEPATITIS B VACCINE: |
Born on or after 1/1/93 |
Three doses. Dose 1 and 2 must be separated by at
least 28 days.
Dose 3 separated by a minimum of 4 months from dose 1, and 2 months from dose 2, and administered on or after 24 weeks of age. |

VARICELLA (CHICKEN POX) VACCINE |
K through 3rd grade |
One dose or history of disease as reported by parent or health care provider. |
|
6th - 9th grade |
One dose, unless administered > 13 years of age then
two doses.. Or history of disease as reported by parent or health care provider. |

Acceptable intervals: |
Dose 1 administered on or after 12 months of age. Dose 1 and 2 separated by at least 28 days. |
For all minimum intervals and age requirements, a 4-day grace period is acceptable.
The vaccines and doses above are the minimum requirements for school attendance.
