The following forms are available for you to print and use as needed. 

Change of Dismissal Forms
These forms can be used to inform us of changes in your child's dismissal plans.  Please note that written notification is required for all changes of dismissal plans and must be provided to us no later than 2:45 p.m.

Placement Information Request
This form is used by parents/guardians who wish to provide us with information you believe might help us place your child in the most appropriate classroom setting.  Please note that we cannot honor specific teacher requests.

Transfer of Pupil Record Form
This form needs to be completed when you have a student who is transferring from another school to Bakie School.

Internet Access Permission Form
